1. Review the DBE Georgia Uniform Certification Application Instructions

2. Review the DBE Georgia Uniform Certification Application Supporting Document Checklist
   • Download the Supporting Document Checklist and check all items that apply to you and your firm.
     Note: The checklist does NOT have to be submitted. However, in order to complete your application for DBE certification, you MUST attach copies of all of the
     documents listed in the checklist as they apply to you and your firm.
      
3. Complete the Uniform Certification Application
   • Download the Georgia Uniform Certification Application
      
4. Save your completed PDF Application and submit
   • Email your application to applications@gdotsupportiveservices.com
   • Or mail to:    GDOT Supportive Services

5. Complete the Affidavit of Certification Form
   • Download the Affidavit of Certification and complete the form.
   • Notarize the Affidavit
